In order to be considered eligible for funding, public library boards must submit four (4) documents as part of their grant application:

1. 2009 Public Library Survey and 2008 Annual Report of Public Libraries in Alberta - Deadline: FEBRUARY 28, 2009
2. 2008 Statement of Receipts and Disbursements - Deadline: JUNE 15, 2009
3. 2009 Budget - Deadline: JUNE 15, 2009
4. Application for Financial Assistance - Deadline: JUNE 15, 2009

In addition, the library board must receive a local appropriation from council of at least $2.00 per capita and be in compliance with the Libraries Act and Libraries Regulation.

Final application deadline is June 15, 2009 but applications will be processed as soon as they arrive. The application will not be considered to be complete if all documents are not received. Determining which version of the forms to use:

• If you want to fill in the forms electronically, use the Word, PDF, or online format. (Online format only available for the Annual Report)
• If you want to fill in the form electronically and have it automatically calculate totals, use the Excel format. (Excel format only available for Statement of Receipts/Disbursements and Budget)
• If you wish to fill in the form manually, use the PDF format. (Right click and select "Print Target").
• Forms filled in electronically should be printed when completed for your board’s records.
