Financial Reporting Forms

In order to be considered eligible for funding, library boards must submit the required documents by June 15, 2009.

These forms are for the exclusive use of public library boards in Alberta as established under the Libraries Act.


NOTE:  Library Boards serving populations under 10,000 are asked to use the provided forms (see below).  Boards serving larger populations may submit their own budget and audit (original documents only).

2008 Statement of Receipts and Disbursements (or audit)

The 2008 Statement of Receipts and Disbursements or audit must be completed by a person or firm approved by municipal council. Submit original, signed documents only; photocopies will not be accepted.  If the auditor is not the same as last year, enclose a copy of the council resolution authorizing the new person or firm as auditor.

2009 Budget

To qualify for the annual operating grant, the library board must receive a local appropriation from council of at least $2.00 per capita.  If the Municipal Council pays operating costs in addition to a cash payment to the board, please have the municipality complete page 7 of the Statement of Receipts and Disbursements and page 5 of the Budget.  Figures included on these pages should NOT be included elsewhere on the forms.

Application for Financial Assistance

The application form must be signed by a member of the library board or authorized library staff member and be counter-signed by a Commissioner for Oaths (please include expiry date).
