Survey and 2008 Annual Report Form
In order to be considered eligible for funding, library boards must complete and submit this report by February 28, 2009.
Public Library Survey
In order to be considered complete, all sections of the Public Library Survey (pages 1-3) must be filled in. When listing board members, please list current members, their addresses, phone numbers and expiry date of their terms. Municipal Boards must have at least 5 members and no more than 10. No more than 2 may be municipal councillors.
